Why Retirees Need Private Health Insurance in Spain
NLV holders are not entitled to access the Spanish public health system via social security contributions (as they're not working). They must maintain private health insurance throughout their visa period — this is both a visa requirement and the practical means of accessing healthcare.
For UK nationals who are state pensioners, there is an important exception: the S1 form (see below) may give access to Spanish public healthcare — potentially replacing the need for private health insurance.
The UK S1 Option for British Retirees
UK state pensioners living in Spain can obtain an S1 form from the UK Department for Work and Pensions (DWP). This form, when registered with the Spanish health authorities, entitles the holder to Spanish public healthcare at the UK's cost. For eligible UK retirees, this can reduce or eliminate the need for private health insurance, though many still maintain a private plan for faster access and English-language service.
Note: you must be receiving a UK state pension to qualify. Private pension income alone is not sufficient.
Costs for Retirees
Health insurance becomes significantly more expensive with age. Approximate monthly premiums for a no-copayment plan in 2026: age 55–60: €180–€280/month; age 60–65: €230–€380/month; age 65+: €300–€500+/month. These costs should be factored into your retirement income planning alongside the NLV income requirement.

What the NLV Health Insurance Must Include
The health insurance for an NLV application must: cover Spain for the full visa duration, provide full medical coverage (not just emergency), have no copayments or co-insurance, and be from a provider recognised as operating in Spain. The policy must also cover each family member included in the visa application.
