Spain’s immigration regulations require student applicants to hold health insurance with an insurer authorised to operate in Spain, giving cover similar to the Spanish public health system (Royal Decree 1155/2024, art. 35).
The Ministry’s interpretative criteria for study authorisations (updated 7 August 2025) add the practical detail. The insurer must be listed in the Spanish insurance supervisor’s register, the cover must be comprehensive, and the policy must have no co-payments, no refund-only cover and no waiting periods, staying in force for your whole stay.

Your cover should start when you arrive and run until your stay ends. If you extend your stay for another academic year (a prórroga de estancia por estudios), your insurance needs to continue for the extended period too.
If you work while studying
Where a student works under the study rules and is registered with Spanish Social Security, the regulations treat that registration as meeting the health insurance requirement (art. 57.5). This applies only while you are working and registered.
Family members
Family members who join a student in higher education need their own cover, named on the certificate.
